The Saline filled breast implant is an implant that is filled with a sterile saline solution - the same type of solution as the salt-water solution that is administered for intravenous fluids. The major advantage of using this type of breast implant is its safety. Should a saline implant become compromised and leak, the saline solution is absorbed by your body, the same as the intravenous fluids used during and after a surgical procedure. Another great advantage of using saline implants is that they offer a low infection rate, usually less than one to two percent of all breast augmentation surgery cases.

Silicone implants are filled with a silicone gel. Approximately 1.5 million women in America currently have silicone breast implants. Some feel that a silicone implant offers a more natural appearing breast. The one main disadvantage of using silicone breast implants is that any possible rupture of the implant is not harder to detect than a ruptured saline implant. When a saline implant ruptures, the salt water solution is absorbed into the body, which results in the breast going flat, which is easily recognized.

In contrast, the silicone gel of a ruptured implant stays within the breast tissue and is not absorbed into the body, there is usually no readily recognizable reduction in breast size after a silicone breast implant ruptures. The tools a plastic or cosmetic surgeon can use to detect the silicone implant rupture include a Mammogram and Magnetic Resonance Imaging, more commonly known as an MRI. The clean up removal process of silicone gel from a ruptured silicone breast implant is difficult and time consuming.

Finally, Cohesive gel implants, also known as "gummy bear" implants, are silicone implants that comprised of a thicker silicone gel that holds its shape and form in the same way as a "gummy bear" candy. If you were to cut open a standard silicone breast implant, the silicone fill gel leaks out of the implant shell. When cutting open a cohesive gel implant, the implant itself maintains its shape and the gel does not leak out. These pre-shaped breast implants are designed with a natural look in mind, rather than for enlarging the breasts. Furthermore, the Cohesive gel breast implant maintains its shape in an upright position, decreasing the chances of the breast implant shell folding or rippling.

The breast implant can be placed either partially under the pectoralis major muscle (submuscular) or on top of the muscle and under the breast glands (subglandular) depending on the thickness of your breast tissue and its ability to adequately cover the breast implant. You should discuss with your surgeon the pros and cons of the breast implant placement selected for you.

The submuscular placement may make surgery last longer, may make recovery longer, may be more painful, and may make it more difficult to have some reoperation procedures than the subglandular placement. The possible benefits of this placement are that it may result in less palpable implants, less capsular contracture, and easier imaging of the breast with mammography. The subglandular placement may make surgery and recovery shorter, may be less painful, and may be easier to access for reoperation than the submuscular placement. However, this placement may result in more palpable implants, more capsular contracture, and more difficult imaging of the breast with mammography.
